AGI plus tax-exempt interest from your 2025 return.

Uses projected 2027 Medicare figures (2026 Trustees Report). Official CMS amounts are due in mid-November 2026.

How this calculator works

Most people pay the standard Medicare Part B premium. If your income is above certain levels, you pay more through an Income-Related Monthly Adjustment Amount, known as IRMAA. This calculator shows which 2027 premium tier your income would place you in and how much extra that adds up to over a year.

Important: the 2027 premium and bracket figures here are projected, based on the 2026 Medicare Trustees Report. CMS normally announces official 2027 amounts in mid-November 2026. For comparison, the official 2026 standard premium is $202.90 a month.

The inputs. Choose your filing status and enter your 2025 modified adjusted gross income (MAGI). Medicare uses a two-year lookback, so your 2027 premium is based on the tax return you file for 2025. MAGI here means your adjusted gross income plus any tax-exempt interest.

The method. The tool matches your MAGI to the projected tier below, then calculates the annual surcharge per person as (your tier premium − $209.50) × 12. For married couples who both have Medicare, each spouse pays the tier premium.

Single (2025 MAGI)Married filing jointly (2025 MAGI)Projected 2027 Part B per person
$112,000 or less$224,000 or less$209.50
$112,001–$141,000$224,001–$282,000$293.30
$141,001–$176,000$282,001–$352,000$419.00
$176,001–$211,000$352,001–$422,000$544.70
$211,001–under $500,000$422,001–under $750,000$670.40
$500,000 or more$750,000 or more$712.30

Married people who file separately and lived with their spouse during the year have only three tiers: $112,000 or less pays the standard premium, $112,001 to under $388,000 pays $670.40, and $388,000 or more pays $712.30.

Worked example. George is single and his 2025 MAGI was $150,000, largely because he sold some investments that year. That puts him in the $141,001–$176,000 tier, with a projected 2027 Part B premium of $419.00 a month.

  • Monthly surcharge: $419.00 − $209.50 = $209.50.
  • Annual surcharge: $209.50 × 12 = $2,514.
  • Total projected Part B for 2027: $419.00 × 12 = $5,028.

Note that IRMAA tiers work like a cliff: going $1 over a threshold means paying the full higher premium. Large one-off income, such as a Roth conversion, a property sale or an extra IRA withdrawal, can push you into a higher tier two years later. Our RMD calculator can help you plan required withdrawals, and if your premium is deducted from Social Security, the COLA calculator shows how it affects your net check.

Part D and appeals. If you have Medicare drug coverage, an income-related surcharge also applies to Part D and is paid to Medicare, not to your plan. This tool does not calculate the Part D amount. If your income has dropped because of a life-changing event, such as retirement, the death of a spouse, divorce or loss of a pension, you can ask SSA to use more recent income by filing Form SSA-44.

Limits of this estimate. The tiers and premiums are projections until CMS confirms them. The tool does not include the Part B late-enrollment penalty (10% for each full 12-month period you could have had Part B but did not), Part D premiums, or Medicare Advantage plan premiums. Our 2027 Part B premium and IRMAA guide will be updated when official figures are released.

Frequently asked questions

Why is my 2027 premium based on 2025 income?

Social Security uses the most recent tax return the IRS provides, which for 2027 premiums is usually your 2025 return. That two-year gap is why a recent drop in income may not be reflected yet.

How do I appeal an IRMAA decision?

If a life-changing event reduced your income, file Form SSA-44 with Social Security along with proof of the event and your new income estimate. You can also request a reconsideration if you think the tax information used was wrong.

Do both spouses pay IRMAA?

Each spouse who is enrolled in Part B pays the premium for the tier set by the couple's joint MAGI. If both are enrolled, both pay the higher amount.

Are these 2027 numbers final?

Not yet. They are projections from the 2026 Medicare Trustees Report. CMS usually announces final 2027 premiums in mid-November 2026.
